Surgical Scars and Keloids: How Scar Lasers Approach Treatment
Most surgical scars gradually fade over time, but some become red, raised, or firmer than the surrounding skin. When this raised tissue continues to grow beyond the original incision line, it is classified as a keloid; when the thickening stays within the incision boundary, it is called a hypertrophic scar. Both are understood to result from excessive collagen production during wound healing, though the exact mechanism is not yet fully understood. Genetic tendency, tension at the wound site, and the degree of inflammatory response are reported to play a combined role.
Why do scars heal so differently from person to person?
Even with the same procedure on the same body area, scarring outcomes vary widely between individuals. People with darker skin tones, a family history of keloids, or wounds located in high-tension areas (chest, shoulders, jawline) are generally reported to have a relatively higher likelihood of raised scarring. In contrast, low-tension areas like the eyelids tend to heal with thinner scars. How do scar lasers work?

Once the scar tissue has stabilized somewhat, fractional lasers such as Fraxel may be considered to create controlled micro-injuries that encourage collagen realignment. Fractional lasers are reported to help improve surface texture and firmness of scars, but they are not a treatment that removes scars entirely, and individual results vary considerably.
Can keloids be treated with laser alone?
Keloids are generally managed with a combination of approaches rather than laser alone. Intralesional steroid injections are commonly used to reduce the volume of overgrown tissue, pressure therapy or silicone sheets help regulate tension, and when appropriate, keloid treatment is used to build a comprehensive management plan suited to the individual case. Laser is often used as a supportive tool within this combined approach, mainly to help adjust scar color or vascularity, and is unlikely to resolve a keloid on its own. Because recurrence is possible, regular follow-up after treatment is important.
When should surgical scar management begin?
The right time to start scar management depends closely on how mature the scar is. During the acute phase, before the wound has fully healed, protecting the wound and sun protection generally take priority over aggressive laser treatment. In the first few weeks to months after stitches are removed, when the scar tends to be red and raised, vascular laser or lower-intensity care is often introduced. Once the scar has matured, turned pale, and the tissue has stabilized, fractional laser aimed at improving texture may be considered. That said, timing and method vary depending on scar location, size, and individual skin response, so a uniform approach is difficult to apply.
What should you keep in mind during treatment?
Areas treated with scar lasers can be particularly sensitive to pigmentation changes, so thorough sun protection before and after treatment is recommended. Temporary redness or scabbing may occur after laser sessions, and it's best to avoid friction or excessive irritation to the area during this period. Depending on the scar's condition, treatment may require multiple sessions, with intervals and frequency often adjusted based on how the scar responds over time.
